There is a one time non-refundable $75 dollar enrollment fee per person to cover the administration costs of onboarding. This is part of the membership enrollment process.

Sustainability Membership
If you feel strongly about equitable access to quality health care, and you have the means to support it, please consider adding $20 per month to your rate - you can let us know at your Onboarding Visit. This will allow us to offer care to members of our community with fewer resources.

Phone, text, or secure messaging in the format that works best for you.
In-Office Procedures
Minor procedures performed in the office when appropriate, including wart and skin tag removal, skin biopsies, simple laceration repair, joint injections, IUD and implant placement and removal.
Coordinated, Patient-Centered Care
Guidance navigating referrals, specialists, and follow-up care.
Discounted Labs, Medications, & Imaging
Transparent cash pricing when insurance coverage is limited or complicated.
Unlimited Primary Care Visits
Routine check-ups, well woman care, well child care, peri-menopausal care.
Extended visits allowing for comprehensive preventative health counseling and thoughtful, ongoing care for long-term conditions- all with YOUR doctor.
What is not covered under your membership?
Specialist visits
Labs, imaging, and procedures performed outside the clinic
Hospitalizations, emergency care, and surgeries
Medications
Any service billed by outside facilities or providers
These costs can be paid through insurance or cash pricing, depending on what works best for you.
Billing & Payment
Payments
You will be billed monthly via credit card or direct deposit. The first payment will be billed at the time of the first scheduled visit.
HSA Eligibility
Beginning January 1, 2026, HSA funds may be used to pay membership fees up to $150 per individual per month or $350 per family per month.
FSA Reimbursement
Many FSAs allow reimbursement for Direct Primary Care membership fees. Please confirm eligibility with your employer or HR department before using your FSA to pay for membership
